How to refer someone to us
Make a referral
Please note that we do not accept self-referrals. Referrals to our service can be made by local authorities, statutory agencies, and other recognised support organisations.
To make a referral, please download our referral form, complete it in full, and return it to us either by email or by post using the contact details provided. Incomplete forms may result in delays processing the referral.
